Senior Services Manager

NEBHDCo is seeking a full-time Senior Services Manager (SSM) to provide support services for senior residents in two affordable housing properties, in Richmond Hill (66 units) and Far Rockaway (30 units), Queens, for one of its housing partnerships. Established in 1985, Northeast Brooklyn Housing Development Corporation (NEBHDCo) is a community development corporation based in Brooklyn.

Position Overview:

The SSM's primary responsibility is to provide general services referrals and monitoring, client advocacy and empowerment supports for senior and elderly special needs tenants, that support active living and aging in place. This includes intake and assessment, health, education, social and recreational services. The SSM will work in coordination with property management staff.

Duties and Responsibilities Include:

  • Develop a network of referral linkages and senior community resources for each property
  • Responsible for working collaboratively with partner agencies including developing new partnerships, with the mutual goal of providing connections to safety net and asset building services for residents
  • Provide tenants with up-to-date information and paperwork support regarding programs such as Medicare, Medicaid and other entitlements, and formal supportive and social services
  • Address social and recreational needs of tenants
  • Act as liaison with local care providers including hospitals and long term care providers to ensure successful transitions between facilities as needed
  • Responsible for client records and tenant supports database management for all tenant contacts, both for services implementation quality and for monitoring of data and client reporting requirements of funding sources
  • Work with property management on tenancy issues, including new tenants and special needs referrals
  • Manage expense budget

Requirements:

  • Bachelor's degree in social work, gerontology, psychology, human services, counseling or related discipline preferred; college with appropriate experience considered
  • At least 3 years' experience with care management and social service delivery for seniors and persons with disabilities, preferably in a housing setting
  • Demonstrated knowledge of aging process, aging-in-place needs, senior and disability services
  • Demonstrated knowledge of all relevant public entitlement programs including eligibility
  • Demonstrated knowledge of human services, senior and disability services in New York City, and preferably in the target neighborhoods in Queens
  • Experience working with and knowledge of public funding sources a plus
  • Demonstrated ability to organize, advocate, problem-solve, oversee and provide results for elderly residents
  • Experience organizing and working with volunteers
  • Strong ability to research and manage resource information
  • Excellent planning, organizational and problem-solving skills
  • Valid driver's license and own car
  • Ability to work independently, self-motivated individual
  • Strong written and oral communications skills; ability to interact with tenants, staff, community partners and volunteers, and to promote events
  • Strong computer skills, Microsoft Office Suite; familiarity with services tracking software, e.g. AASC Online
