The Monadnock ServiceLink Resource Center Long Term Care Support Counselor will use person-centered approaches to counsel and connect people 18 and older with long term care options. This includes prescreening for eligibility for state programs and assisting consumers with initiating long term care applications.
The successful candidate will have excellent interpersonal skills and a keen abilty to successfull navigate professional and personal boundaries. Additionally the ideal candidate will have excellent organizational skills with attention to detail; will have skill with and enjoy managing processes; and will be able to work well with multiple health and social service providers and a diverse client population. Ideally, the candidate will have a working knowledge of entitlement programs including Social Security, Medicare and NH Medicaid Programs, and will have experience working with older adults and and/or adults with disabilities.
Finally, the candidate will have a valid driver's license and reliable transportation and will be computer literate in Word and Excel. Bachelor's degree plus 2 years experience. Position offers a competitive salary and comprehensive benefits package.
