Horizon House Inc. has been selected by Philadelphia County to initiate a program supporting outreach, treatment, and housing placements to meet the complex needs of those living on the streets, including successful housing first programs for those with a primary diagnosis of serious mental illness and chronic homeless rehabilitation residences for those who also deal with addictions to drugs. This program seeks to target a population with primary alcohol addictions who have not been successful in any of these interventions. Journey Home will provide supportive housing and treatment for 18 chronically homeless men living with late stage chronic alcohol additions. Participants will be housed in scattered site apartments throughout Philadelphia and will be supported in community.

We are seeking a variety of qualified substance abuse professionals to provide assistance with supportive housing and treatment for 18 chronically homeless men living with late stage chronic alcohol addictions. We are seeking to hire for various positions which include: Team Leader, Drug and Alcohol Specialist, Housing Specialist and a Part time Certified Peer Specialist and a Part time Occupational therapist.

Responsibilities include managing the daily operations of this program designed to assist individuals with histories of chronic homelessness and alcohol abuse to live independently in the community. The Team Leader will ensure that participants receive recovery and housing supports from a multidisciplinary team. This person also acts as a practicing clinician on the team and performs on-call duties. Interested candidate must possess a Bachelor’s degree in Human/Social Services with at least 3 years direct care experience in the behavioral health field and at least 2 years supervisory experience and strong computer skills are essential with electronic medical record systems experience. Master’s degree strongly preferred. Consideration will be given to candidates who are Registered Nurses or Occupational Therapists who meet the above qualifications. Selected candidate must possess a valid driver’s license with an acceptable driving record.

The position provides substance abuse recovery supports, education and counseling to participants. Using multiple service modalities, the Drug and Alcohol Specialist ensures continuity of services, in compliance with funding sources and agency regulations. Responsibilities include assisting staff with the development of strategies for intervening with participants who may have co-occurring disorders. Interested candidates must have either a Bachelor’s Degree in Human/Social Services or a Master’s Degree in Social Sciences and 1-2 years relevant experience in substance abuse counseling. A Certified Addictions Counselor (CAC) credential is highly desirable and computer skills are essential with experience in electronic medical records systems. Selected candidate must possess a valid driver’s license with an acceptable driving record.
