We are looking for compassionate, caring, dynamic staff to provide outreach services to clients living with psychiatric conditions in the Cambridge and Somerville area.

The Clinical and Rehab Coordinators provide expertise and outreach services to adults receiving services under the Department of Mental Health’s Community Based Flexible Supports (CBFS) model and as members of the Collaborative Action Recovery (CAR) Team.

The Clinical Coordinator provides consultation and training to both clients and other team members on mental illness, evidence-based psychiatric treatment and risk reduction strategies. The Clinical Coordinator will complete Assessments, develop Individual Action Plans (IAP) and provide coordination and liaison services for CBFS clients with community and hospital resources.

The Rehab Coordinator provides consultation and training to both clients and other team members on completing functional and strengths-based assessments and implementing Individualized Action Plans (IAP) using rehabilitation values, principles and techniques, including skills training. Assists team members to maintain adherence to service delivery and documentation standards. The Rehab Coordinator also assists the CBFS Team Leader in maintaining tracking systems to support adherence to state and federal documentation standards for clients served by the team.

These are fast paced positions geared towards helping clients engage in the recovery process and become a part of their community. Using a motivational enhancement and strengths based rehabilitation approach, these positions focus on engaging clients and helping them evaluate their current life circumstances compared to their hopes, dreams and plans for the future.

To succeed in this role you must have strong time management, writing and organizational skills. You must also have 3-5 years human services experience, a Master’s degree in Social Work, Psychology or related field required along with licensure or license eligibility (LCSW, LICSW, LMHC, CRC). Candidates for this position must be at least 21 years of age and have held a valid US driver’s license for a minimum of 1 year and be able to satisfactorily pass both a driving check and a CORI check. Additionally, this position requires a fully insured vehicle that you will use in the course of your daily work. You must be willing to transport clients in your vehicle.
