Casa Mutua, located on East 102nd Street, is our 54-unit permanent supportive housing residence for formerly homeless single adults with histories of mentally illness and substance abuse. We are seeking a Clinical Case Manager to join oursocial service team to promote the independence and stability of a caseload of tenants by providing comprehensive supportive services.
Reporting to the Director of Social Services, the Clinical Case Manager will be responsible for providing direct service to a caseload of tenants including assessment, supportive counseling, case management, advocacy for entitlements and services, group work and referral to resources including psychiatric and medical care, civil legal services, substance abuse services, etc. The Clinical Case Manager will be expected to participate in a teamwork approach to service provision, and work cooperatively with staff at all levels at Casa Mutua and throughout Lenox Hill Neighborhood House to ensure quality service provision. The Clinical Case Manager will also be responsible for completing comprehensive assessments to inform, develop and implement service plans, participating in case conferences and staff meetings, escorting tenants to appointments as needed, assisting tenants in problem-solving and conflict resolution between neighbors or outside family/friends and attending and facilitating group activities including monthly parties, meals and groups to model appropriate social behavior and build relationships with tenants. The Clinical Case Manager will be expected to perform other related duties incidental to the work as assigned.
The successful applicant must possess excellent interpersonal and relationship building skills. A social justice perspective, relevant experience and commitment to working with mentally ill and homeless populations are also required. BA or BSW accepted but MSW preferred. The ability to communicate effectively in Spanish is also strongly preferred.
