Case Manager/Mentor Coordinator

Just A Start House is presently seeking A full-time Case Manager/Mentor Coordinator. Bachelors eligible/Masters preferred. Just A Start House is a Teen Living Program serving young mothers and children.

Duties and Responsibilities: Case Manager: Provide case management services to pregnant and parenting teenagers, individually and in groups, within a residential setting. Develop individual service plans as related to education, benefits, and mental health needs. Responsible for skills development with individual residents, available to respond to the needs of individual residents and to respond to crisis. Works one-on-one and in groups with all of the families to teach and role model parenting skills and alternative discipline methods, time management skills, self-advocacy skills, and other lifeskills per PAYA standards. Maintains resident files including weekly progress notes per specified time guidelines. Serves as a point person to coordinate residents’ services. Provides referrals to and assistance in accessing appropriate supportive services. Assists with residents transition planning from the shelter into the community. Milieu support.

Mentor Coordinator: Recruite volunteers to become mentors through outreach efforts. Coordinate training for mentors in the areas of adolescent development, parenting, and child development issues, homelessness, career choices, and transitioning into independant living. Screen potential mentors during training process to discern appropriateness of matching particular volunteers with clients. Match mentors with Just A Start House residents and participants of the Futures for Young Parents Program. Schedule mentors to work with families, and ensure that they are meeting regularly with clients and that a supportive ongoing relationship is developing and being maintained. Provide regular follow-up support and training to mentors in both group settings and individually. Maintain the administrative paperwork necessary to ensure success of the Mentor component of the program. Good organization and communication skills. Experience with adolescents and/or residential programming. Knowledge of parenting. Energetic, open to learning, and good sense of humor. Strong team player.

Schedule is as follows: Monday 10:30 am-7:00pm Tuesday 1 pm-10 pm Wednesday 10 am-6:30pm Thursday 10 am-6:30pm Friday 9 am-5:30pm

Mentor Trainings are held approximately every three months; which requires the Mentor Coordinator to work Monday evenings for five consecutive weeks. Schedule during this time will be changed to allow for this training to be conducted.
