The Community Care Coordination Program in the Positive Living Department seeks two (2) Case Managers to work in an intense medical case management program. The position requires providing direct service to clients including intake, engagement, assessment, creating service plans, making referrals, advocating for clients and acting as a liaison between clients and health and social service providers and agencies; field work, outreach, heavy documentation, and case conferencing with also be necessary.

The ideal candidate will be a self-starter, highly organized, and skilled at documentation. The minimum requirement is a Bachelor’s degree in a related field. Bilingual skills in English and Spanish are preferred.
