Case Manager

Located on the Lower East Side, Los Vecinos Apartments is a permanent SRO housing facility with a capacity for 35 formerly homeless men and women suffering from mental illness, substance abuse and HIV/AIDS. The program is funded by the NYC Department of Homeless Services, the NYC Department of Housing Preservation and Development, the NYC Department of Health and Mental Hygiene and the NYC Human Resources Administration’s HIV/AIDS Services Administration. In addition to providing permanent housing for its tenants, Los Vecinos offers a variety of services aimed at helping clients stabilize their health, maintain their independence and build a community of support among peers. These services include case management, referrals to medical facilities and assistance managing health conditions, medication monitoring, health education, psychiatric management, entitlement support and money management, recreational activities, assistance with activities of daily living and free meals.


Organize recreational activities, conduct a weekly group, and counsel tenants on use of leisure time. Complete service plan reviews and other clinical assessments. Write progress notes, assist with maintaining clinical charts. Carry a caseload, conduct home visits, maintain entitlements, escort clients, and liaise with outside providers. Accompany clients on activities. Provide emergency first aid/CPR assistance when needed. Related duties as assigned.

Full-time position 37.5 hours per week
Sunday, Monday, Tuesday, Thursday, 9:00am-5:30pm
Wednesday, 11:00am – 7:30pm


BA preferred. Two years experience in organizing support groups/recreational activities, client advocacy, coordinating case conferences, and counseling for mentally ill and HIV/AIDS population. NYS driver’s license preferred. CPR training certification or willingness to take training class in CPR.
