Lenox Hill Neighborhood House is a preeminent provider of older adult services. Our ultimate goal for our older adult clients is to help them improve their quality of life, retain their independence and strengthen themselves and their community. We provide a broad range of integrated services for more than 10,000 older adult residents of the community each year, the majority of whom live on low-level fixed incomes. These programs include an intensive case management program that serves more than 1,400 frail and often homebound elderly living on Manhattan's East Side and East Harlem; two active, engaging senior centers; a social adult day program for people living with dementia; transportation services for seniors; and more.
We are seeking a Case Management Social Worker to join us and help us develop and implement a Care Transitions Initiative in partnership with highly-regarded hospital and other critical partners. Working as part of a team, the Case Management Social Worker will assess the needs of older adults, develop a care plan to help meet these needs, and implement the care plan, which includes providing information on appropriate services and programs and making referrals for services as needed (many which are specific to the psychiatric and dementia needs of our clients). The Case Management Social Worker will also assist with the collection of documents and complete necessary paper work for benefits, providing linkages with other services providers and assist with bill paying and health insurance reimbursements. Other responsibilities will include:
- Conducting case management program intake, assessment and referral and becoming knowledgeable about the many resources for elders in the East Side community.
- Serving as a liaison between case management clients and their meals-on-wheels providers.
- Participating in case conferences as an advocate for the client as needed.
- Maintaining statistical, narrative and financial records including written records of contact with clients
- Work closely with hospital staff on providing the best possible care for clients leaving the hospital and work to develop a preventative program that will help clients stay out of the hospital for the long term following a discharge.
We are interested in talking with candidates with high quality comparable experience working with older adults and have expertise in addressing the varied and complex needs of older adults. The Case Management Social Worker must be detail oriented, have significant initiative and also have excellent writing, communication, computer and organizational abilities. The successful candidate must have an LMSW. A competitive compensation package will be provided.
