ACCESS (LICSW/LCMHC) Team Clinician (Full and Part-time)

Job Summary: Demonstrates the ability to conduct crisis intervention assessments on inpatients and outpatients utilizing appropriate crisis intervention techniques to stabilize patient and family. Determines the appropriate level of care based on the assessment and coordinates patient disposition. Conducts insurance precertification in a timely manner. Work in collaboration with the healthcare team; local hospitals; and other healthcare agencies.

Responsibilities:
• Demonstrate the ability to conduct crisis intervention assessments on inpatients and outpatients
o Perform mental status examination
o Evaluate patient's level of use and/or dependence on drugs of abuse
o Determine patient's level of safety
o Conduct psycho-social evaluation
• Utilize crisis intervention techniques to stabilize patient and family
o Respond to patient situation
o Employ de-escalation techniques
o Handle patient in a controlled and calm manner
o Facilitate patient safety
• Determine appropriate level of care based on assessment
o Establish tentative diagnosis
o Make appropriate contact with appropriate family members
o Collaborate effectively with outside providers such as primary care physicians, psychiatrists, and therapists
o Communicate accurate verbal and written description of the patient's conditions to the appropriate provider and other health care team members
o Develop preliminary treatment plan
• Conduct insurance precertification
o Document communication with insurance carriers
o Carry out precertification process
o Follow procedures to transfer incomplete precertification
• Coordinate patient disposition
o Provide safe hand-off to appropriate facility
o Arrange transfer to recommended facility
o Facilitate involuntary emergency admission
o Provide patient education

Qualifications:

Educational and Professional
• Licensed in New Hampshire for Clinical Mental Health Counselor (LCMHC) or Licensed Independent Clinical Social Worker (LICSW) Required
• Master's Degree in human service field

Knowledge/Abilities/Requirements
• Excellent communication skills required
• Experience, education and/or training in social work, counseling, psychology or other related human service field
• Able to express or exchange ideas by means of the spoken word. Primary functions include activities in which incumbent must convey detailed or important spoken instructions to include hearing conversational tones, phone message, stethoscope, etc.
• Ability to maintain physical demands of bending, standing, walking, reaching, pushing, pulling, lifting and carrying up to 50 lbs
• Full and Part-Time to include evening shifts as well as nights and weekends
