Financial Aid Application Checklist, 2010—2011

Before completing the checklist below, click here for general information on loans and determining your loan amount.

All Students New and Continuing MUST Complete:

  1. Free Application for Federal Student Aid (FAFSA) 2010-2011
    Be sure to include our school code (E00557) on your FAFSA. If you have questions call (800) 433-3243.
  2. Financial Aid Request Form, 2010-2011 (PDF 103K)
    Use this form to request student loans and work study. Mail or fax to our office.
  3. 2009 Federal Tax Return
    Mail or fax a signed copy of your 1040 (1040, 1040A, 1040EZ, 1040PC or Telefile) to our office.

    A limited number of students may be selected for a process called verification. Those selected will need to submit the Verification Worksheet, 2010-2011 (PDF 66K). The Financial Aid Office will notify you if you have been selected.
  4. Federal Direct Loan Master Promissory Note
    Sign in using your PIN and select the first option, "Subsidized/Unsubsidized."

Important Notice: Beginning with the Summer Term for the 2010-2011 academic year, Antioch will be using the Federal Direct Loan Program ONLY. Continuing Antioch borrowers will be required to complete a new Master Promissory Note with Federal Direct Loans before any funds will be disbursed.

First time Antioch Borrowers

First time Antioch Borrowers must complete the Direct Loan Entrance Counseling.

In most instances, students applying for loans only can complete the above application process 3 weeks prior to registration.

New Summer Students
New Fall 2010 PsyD Students
 June 1, 2010 New Fall 2010 Students
 November 1, 2010 New Spring 2011 Students
