Vision 2020 is being implemented in three phases.
Phase I: Getting It Right Inside: Achieving Service Excellence (2006-2009)
Phase I is under way. Its purpose is to assure ready access to primary care; make it easier for patients to receive care and communicate with CMC/DHK; measure and continuously improve quality and services; conduct needs assessments for our patients, community and organization; survey patient and employee satisfaction and act on results; identify measurement tools for healthiest community. During Phase I, CMC/DHK is focusing on the practices and procedures that enhance quality care delivery. One objective is to provide patients with a consistently good experience. Every one of CMC/DHK’s nearly 1,400 staff members believes that what they do plays an integral part in the realization of Vision 2020.
Phase I “GIRI” – Year 3 (2008-2009)

Phase II: Leading the Way (2010-2014)
Pull together community partnerships and resources and provide leadership to become the nation’s healthiest community.
Phase III: Transforming (2015-2020).
Set an example for other communities to emulate and share expertise; become an acknowledged leader in developing innovative approaches to community health improvement.
